V Nail

The V-Nail is a type of intramedullary fixation device characterized by its V-shaped cross-sectional design, intended primarily for the treatment of long bone fractures, especially the femoral shaft. It is a variant of the classical Küntscher Nail (K-Nail) and offers improved mechanical stability and anti-rotational properties due to its geometric profile.
Manufactured from stainless steel or titanium, the V-Nail is typically a solid, non-cannulated implant, inserted into the medullary canal of the bone through an antegrade approach. While traditional V-Nails do not feature locking options, some modern versions may include proximal and distal holes to accommodate interlocking screws, enhancing fixation in unstable fractures or osteoporotic bone.
Its V-shaped structure allows better press-fit engagement with the endosteal surface, thereby increasing the rotational and axial stability of the fracture construct. The V-Nail is valued for its simplicity, durability, and affordability, especially in settings where cost or access to advanced implants is a concern.
- Midshaft femoral fractures: Particularly transverse or short oblique fractures with minimal comminution.
- Diaphyseal fractures of long bones: Including femur, tibia (in selected cases), and possibly humerus where intramedullary support is appropriate.
- Fractures in low-resource or rural settings: Where advanced locking nails may not be available or feasible.
- Simple, closed fractures: Especially when surgical instrumentation and imaging support are limited.
- Revision procedures: Can be used in nonunions or failed previous fixations, particularly when rotational control is still achievable without locking screws.
- Pathologic or osteoporotic fractures (select cases): Depending on fracture morphology and stability requirements.
