K Nail



The Küntscher Nail, commonly referred to as the K-Nail, is a straight or slightly curved intramedullary fixation device used primarily for the treatment of midshaft femoral fractures. Introduced by Gerhard Küntscher in the 1930s, it was one of the first devices to use the medullary canal for internal fixation, revolutionizing fracture management.
Made from stainless steel or titanium, the K-Nail is typically solid, with a cloverleaf or V-shaped cross-section, designed to provide a tight press-fit within the canal. It is inserted antegrade through the greater trochanter or piriformis fossa and functions as a load-sharing implant. Though traditional K-Nails lack interlocking screw holes, modern variants may include proximal and distal holes for locking screws to prevent rotation and telescoping.
While newer nailing systems have largely replaced the classical K-Nail in many settings, it is still used effectively in resource-limited environments or for specific clinical indications.
- Isolated midshaft femoral fractures – Especially simple, closed fractures with minimal comminution.
- Diaphyseal fractures of the femur – Providing internal stability along the long bone shaft.
- Fractures in resource-limited settings – Where advanced implants or locking nail systems may not be available.
- Pathological fractures (select cases) – Can be used to stabilize the shaft in weakened bone when locking is not critical.
- Secondary fixation – In some revision procedures or nonunion cases where plating or other methods have failed.
