6.4 Mm Proximal Femoral Screw (P.F.N)

The 6.4 mm Proximal Femoral Screw is a specialized fixation screw used in Proximal Femoral Nailing (PFN) systems for the treatment of intertrochanteric and subtrochanteric fractures of the femur. Made from high-strength titanium alloy or stainless steel, this screw is designed to anchor the intramedullary nail to the femoral head and neck, ensuring rotational and axial stability in the proximal femur.
With a threaded, partially cannulated design, the 6.4 mm PFN screw allows for guided insertion over a guide wire, improving placement accuracy. It is typically paired with a larger lag screw (e.g., 8.0 mm) or helical blade, and works in tandem to prevent rotation and collapse of the femoral head fragment.
The 6.4 mm screw is most commonly used as the anti-rotation (derotation) screw, particularly in dual-screw PFN systems, enhancing the overall biomechanical stability of the implant construct.
- Intertrochanteric fractures of the femur:- As part of PFN systems to stabilize the femoral head and neck fragment.
- Subtrochanteric femoral fractures:- Provides rotational control and support in conjunction with a main lag screw or blade.
- Unstable proximal femur fractures:- Ensures secure fixation in comminuted or osteoporotic bone where rotation control is critical.
- Osteoporotic bone fixation:- Smaller diameter allows less invasive entry into compromised bone while still providing stability.
- Dual-screw PFN constructs:- Used as the anti-rotation screw paired with a lag screw to enhance fracture compression and prevent head collapse.
- Revision surgeries:- Can be used in re-operations to add rotational stability to previously placed PFN systems.
